Regarding outpatient substance abuse treatment and related services, there is a 30 percent coinsurance charge after the deductible has been reached. With regards to inpatient services, there is a $500 copay that will apply after the deductible has been met in addition to the same 30 percent coinsurance charge.

Under the Bronze Pathway X PPO 6400 20 plan, the total estimated monthly premium per individual is $291. The estimated yearly deductible per individual is $6,400, while the estimated out of pocket maximum expense is $6,600. If you live in the state of Mississippi and are seeking outpatient substance abuse treatment services, a 20 percent coinsurance charge will apply after the yearly deductible has been met.
