Blue Cross and Blue Shield of Illinois offers several different types of individual health insurance plans, all of which offer some degree of coverage for inpatient and outpatient substance abuse treatment programs. Catastrophic and Bronze plans generally require greater out-of-pocket costs, while Silver and Gold plans tend to cover a greater portion of treatment expenses.

Catastrophic Coverage

Catastrophic coverage carries the highest deductibles and is available only to people who are under age 30 or who otherwise qualify for a ‘hardship exemption’. There are two catastrophic Blue Cross and Blue Shield plans in Illinois –Blue Choice Security PPO 008 and · Blue Security PPO 010. Both require plan participants to meet their deductible before the plan pays 100 percent of inpatient and outpatient substance abuse treatment costs.

Bronze Coverage

There are several BCBS bronze coverage options available in Illinois. Two plans – Blue Choice Bronze PPO„ 006 and Blue PPO Bronze 006 – pay 100 percent of inpatient and outpatient substance abuse treatment costs after patients meet a deductible. Others boast lower deductibles, but they require covered individuals to pay a patient portion of treatment costs before and/or after the deductible has been met. These include:

Blue Cross Blue Shield Basic 5, a Multi-State Plan – once patients meet a deductible, the plan pays for 70 percent of inpatient and outpatient treatment costs. A $300 copay is applied for inpatient substance abuse treatment services.
Blue Choice Bronze PPO 005 – patients pay a $30 copay for outpatient substance abuse treatment services. Inpatient services are subject to the deductible, after which the plan pays 80 percent of treatment costs.
Blue PPO Bronze 005 – patients pay a $30 copay for outpatient substance abuse treatment services. Inpatient services are subject to the deductible, after which the plan pays 80 percent of treatment costs.
Blue Precision Bronze HMO 003 – outpatient substance abuse treatment requires a $25 copay with each session. Inpatient services are subject to a $300 copay plus deductible, after which time the plan will pay 70 percent of covered treatment costs.

Silver Coverage

Silver coverage requires less out-of-pocket patient costs for substance abuse treatment services than bronze coverage options do. Three of the Blue Cross and Blue Shield of Illinois silver plans require copays only for outpatient and inpatient treatment. These include the Blue Choice Silver PPO 003 and Blue PPO Silver 003, which require a $30 copay for outpatient services and a $250 copay for inpatient treatment, as well as the Blue Cross Blue Shield Solution 4, a Multi-State Plan, which requires a $40 copay for outpatient substance abuse treatment services and a $250 copay for inpatient care.

Four other silver coverage plans from BCBS of Illinois also require a $30 outpatient and $250 inpatient copay, but they cover just 80 percent of inpatient treatment costs after deductible. These include Blue Choice Silver PPO 004, Blue PPO Silver 004, Blue Cross Blue Shield Solution 3, a Multi-State Plan, and Blue Precision Silver HMO 002.

Gold Coverage

BCBS gold plans provide the most comprehensive substance abuse treatment coverage with the least individual out-of-pocket costs. Two plans – the Blue Choice Gold PPO 001 and Blue PPO Gold 001 – offer a $30 patient copay for outpatient services and just a $200 copay for inpatient treatment.
