On that note, deductibles and copays are generally much more expensive than they are with gold and silver plans.

When it comes to inpatient substance abuse treatment, the plan covers it at 20 percent co-insurance for in-network providers only. For outpatient services, there’s a $40 copayment. Neither are covered if an out-of-network provider is selected.

Another offered plan is the Silver $10 Copay AVN HMO, which carries a $3,500 individual deductible and $7,000 family deductible. When it comes to inpatient rehabilitation for substance abuse, the service is covered in-network at 30 percent coinsurance, following a $500 copay that must be provided upon admission.

Lastly, there’s also the Bronze $15 Copay AVN HMO, with $6,850 and $13,700 deductibles, respectively. When it comes to inpatient treatment for substance abuse, the plan covers it at 0 percent coinsurance.
